Child's Passport Kit

Checklist:

• Completed DS-11 passport application form with your child's information. Do not sign application until requested to do so by the Clerk of Court or official Postal Employee.
• Two (2) passport photos.
• Certified U.S. birth certificate (not hospital certificate) or original certificate of citizenship or original certificate of naturalization. Children with expired passports should submit a certified state issued birth certificate along with the previous passport.
• Copy of travel plans; Travel Itinerary,company letter or copy of tickets.
• "Client's Authorization Letter", signed by both parents.
• Completed DS-64 (Statement Regarding Lost or Stolen Passport) if this is to replace a lost or stolen Child's passport.
• Children under 14 years old -both parents and child need to be present at Post Office or Clerk of Court when executing your child's passport application. If only one parent can be present, the parent that does not go needs to complete a DS-3053 application and have it notarized.
• Applicable fees (see instructions sheet).
• For All Minors Under Age 16:

  • Each minor child shall appear in person.
  • All applications for children under 16 require both parents' or legal guardians' consent.
     

  For All Minors Ages 16 & 17:

  • Each minor child shall appear in person.
  • For security reasons, parental consent may be requested.
  • If your child does not have identification of his/her own, you need to accompany your child and
    present identification.
