Adults: First-Time (age 16 and over) = $170 Adults: Renewal = $170 Minors: (under age 16) = $140 Additional Pages: $142 Lost or Stolen Passport For individuals who: Checklist: Completed DS-11 passport application form, do not sign until requested to do so by the Clerk of Court or Official Postal Employee. Two passport photos. 2" x 2" white background. No logos or headgear of any kind. Certified U.S. birth certificate or previously issued passport (must be in good condition, free of any damage) or original certificate of citizenship or original certificate of naturalization. Copy of travel plans. Completed and signed "Client's Authorization Letter". If passport was lost of stolen: completed DS-64 Statement Regarding Lost or Stolen Passport. Completed and signed "Service Order Form". Applicable fees (see instructions sheet).
