STEP 3: Click here to complete the SERVICE ORDER FORM. STEP 4: Send all requirements to our Houston or Miami office via FedEx for visa processing. Important: All applicants must have a valid passport to apply for a visa. Your passport must remain valid for at least 6 months after departing Belarus and must have at least 2 blank visa pages.
