Replace Mutilated Passport For individuals who: Checklist: Completed DS-11 passport application form, do not sign until requested to do so by the Clerk of Court or Official Postal Employee. Two passport photos. 2" x 2" white background. No logos or headgear of any kind. Certified U.S. birth certificate or previously issued passport (must be in good condition, free of any damage) or original certificate of citizenship or original certificate of naturalization. Copy of travel plans. Completed and signed "Client's Authorization Letter". Statement regarding the nature and explanation of the damage/mutilation to the passport. Completed and signed "Service Order Form". Applicable fees (see instructions sheet).
