You may use this option if: You are able to submit your most recent U.S. passport. Your most recent passport was issued in your current name. Your most recent passport was issued within the past 15 years. Your most recent passport was issued when you were 16 years of age or older. Your most recent passport is in good condition, free of any water damage, tears or separations.
Checklist: Completed and signed DS-82 passport renewal form. Two (2) passport photographs, 2" x 2", white background, no logos or headgear, taken within the last 6 months. Your U.S. passport in good condition. Proof of travel: copy of airline ticket, itinerary, or letter from company if travel is for business. Completed and signed "Client's Authorization Letter". Completed and signed "Service Order Form". Applicable fees (see instructions sheet).
